IELTS考试申请表填表分项说明
- 首选考试日期： 请填写考点正式公布可以受理报名的考试日期。不需填写第二选择日期。Select a test date as published by the test centers. You don’t have to make the second choice.
2. Family Name：所有考生使用英文填写考生本人姓氏（SURNAME），本国考生加中文标注。Write your surname here companied it with Chinese character for the Chinese candidates.
3. 圈选相应的个人称谓（Mr/Mrs/Miss/Ms）。Circle the preferred title.
4. Other Name：所有考生使用英文填写考生本人名字（GIVEN NAME），本国考生加中文标注。This is the given name for all candidates. Write them in Pinyin and Chinese characters, if you are a Chinese.
5. 联系地址：所有考生使用英文填写考生本人联系地址和邮政编码，本国考生加中文标注。Address: Chinese candidates must write the address both in English and Chinese.
6. 联系电话：填写个人电话或方便与考生取得联系的电话。The candidates’ telephone number: please leave your personal telephone number or the most reachable phone.
